What you need for this position:

- To be a graduate from an accredited school of Physical Therapy program
- Current Oklahoma Physical Therapist license

What you'll be doing:

- Interpret referrals; examines, evaluates, develops, and modifies physical therapy plan of care including anticipated goals and expected outcomes for patients
- Performing comprehensive physical assessment of assigned patients as evidenced by clinical documentation
- Coordinates patient care with other clinical team members ensuring optimal patient care and communication as noted in the clinical record
- Maintaining and submitting all required patient documentation in a timely manner and in accordance with home health policies and procedures
