Patient Care Coordinator (considering LPN or RN) Job Summary The Patient Care Coordinator (PCC) is responsible for the coordination of care of an assigned group of multidisciplinary patient care teams providing care under the direction of a Field Case Manager. QUALIFICATIONS
* Is currently licensed as a Registered Nurse or LPN in the state of Louisiana.
* Has a minimum of one (1) year of experience as a professional nurse with preferably (1) year experience in the field of Home Health Nursing within the last 2 years.
* Possess knowledge of Medicare and Medicaid guidelines essential to Home Health Coverage.
* Has one(1)year of clinical supervision preferably in the field of Home Health Nursing
* Demonstrates the ability to assess the interpretation and formulation of patient care plans to meet the needs appropriate to the age of the patients serviced by the agency
* Demonstrates proficiency in clinical documentation and strong verbal, written communication and interpersonal skills.
* Coding experience
* Current knowledge in home health regarding the 2008 changes in Medicare & OASIS and coding Director of Nursing Summary: Be knowledgeable and communicate to all staff, in conjunction with the administration, current nursing and medical developments, health care legislation and regulations and administrative policies, changes in federal, state or local regulatory agencies. Requirements:
* Must have Current Louisiana RN license
* Preferably at least 2-3 years of experience in Medicare home health, with at least 1-2 year supervisory experience.
* Have the capacity to supervise, direct and evaluate the performance of all staff, particularly the clinical staff.
* Knowledge and competency to perform required documentation and patient care procedures of a registered nurse and deal effectively with various demands of the job.
* Experience with point of care documentation system preferred
